Partially true, but they are not just guesses either. They are assessments based on a variety of input: body language, facial expression, conduct, words spoken, words not spoken, tone, and other behavioral cues balanced against a known baseline of that person's normal every day or resting behavior or what is known to be generally normal at large.

It all happens in the background, mostly subconsciously but it's still an assessment more than a guess.
